These Terms of Service ("Terms") govern your access to and use of the Sport Screening portal at screeningsports.com and related services (the "Service"), operated by Phuel Sports Science ("Sport Screening," "we," "us," or "our"). By creating an account or using the Service, you agree to these Terms. If you do not agree, do not use the Service.
The Service provides a secure portal through which athletes and their guardians can create an account, complete required consents, and receive health-screening and test results. Specific testing services are described at the point of enrollment and may vary by client organization.
You must be at least 18 years old to create an account. Accounts for minor athletes must be created and managed by a parent or legal guardian, as further described in our Notice of Privacy Practices and Eligibility Policy.
Access to the Service uses passwordless authentication. When you create an account or sign in, we send a single-use, time-limited verification code (OTP) to the email address — and, if you provide one, the mobile number — associated with your account. You are responsible for keeping your contact details current and for all activity that occurs under your account. Do not share verification codes with anyone.
If you provide your mobile phone number and consent at sign-up, you agree to receive one-time verification codes (OTPs) from Sport Screening by text message (SMS). The following terms apply to that program:
You agree not to: (a) access another person's account or data without authorization; (b) attempt to probe, scan, or circumvent the Service's security or authentication; (c) use the Service for any unlawful purpose; or (d) interfere with or disrupt the integrity or performance of the Service. We may suspend or terminate access for violations.
The Service delivers test results and related information but does not itself provide medical advice, diagnosis, or treatment. Results should be reviewed with a qualified healthcare provider. Nothing in the Service creates a physician-patient relationship except as separately established with your ordering provider.
Your use of the Service is also governed by our Notice of Privacy Practices, which describes how we collect, use, and protect your information, including protected health information (PHI) under HIPAA.
The Service, including its software, design, and content, is owned by Phuel Sports Science or its licensors and is protected by applicable intellectual-property laws. You may not copy, modify, or distribute any part of the Service without our prior written permission, except for your own records and results.
The Service is provided "as is" and "as available" without warranties of any kind, to the fullest extent permitted by law. To the maximum extent permitted by applicable law, Phuel Sports Science will not be liable for any indirect, incidental, special, or consequential damages arising from your use of the Service. Nothing in these Terms limits any liability that cannot be limited under applicable law.
We may update these Terms from time to time. The revised Terms will be posted on the Service with an updated effective date. Your continued use of the Service after changes take effect constitutes acceptance of the revised Terms.
These Terms are governed by the laws of the United States and the state in which Phuel Sports Science is organized, without regard to conflict-of-law principles.
